If you do not have a gold star on your license, additional documentation may be required. Review what to bring to determine if any additional documents are needed.


New, expired or renewal: HAZMAT-endorsed CDL applicants must pass the HAZMAT endorsement written test, be fingerprinted, pass the required national-level background check, and present proof of residency prior to receiving the hazardous materials endorsement.

Attention first-time HAZMAT applicants: The Entry Level Training (ELDT) requirement must be verified before drivers may take the HAZMAT endorsement knowledge exam. For more information on ELDT regulations, please visit the Carrier Safety Administration webpage.


Federal Motor Carrier Safety Regulations now require all drivers obtaining an original CDL, or renewing their CDL, to certify with the driver license office in one of four categories. Drivers certifying in non-excepted interstate or non-excepted intrastate commerce must also provide valid medical documentation.

Effective 12/16/25, FLHSMV can no longer accept paper medical certificates. Your medical examiner must submit the DOT medical exam information electronically.
