• Temporary permits renew every six months and cost $15.
• Disabled parking permits can be obtained by mail or in person at multiple branches.
• To complete your transaction in person, complete form 83039 and schedule an appointment. Appointments are recommended but not required.

1. Expectant mother parking permits are valid up to one year and cost $15.
2. Form 83040 must be completed by a physician.
3. Expectant mother parking permits can be obtained by mail or in person at multiple branches.

Once the disabled or expectant mother parking permit is completed and signed by a physician, mail it with a copy of our valid State driver license or identification card to:
